Pediatric Symptom Checklist-17 (PSC-17)

The Pediatric Symptom Checklist-17 is a brief screening questionnaire designed to facilitate the recognition of cognitive, emotional, and behavioral problems in children ages 4 to 17. The questionnaire is to be completed by biological, adoptive, or guardian of the child.

Instruction:

Emotional and physical health go together in children. Because caregivers are often the first to notice a problem with their child’s behavior, emotions or learning, you may help your child get the best care possible by answering these questions.
